• <br />ISSUED TO: <br />LOCATION: <br />HOURS OF OPERATION: ) <br />E• ; CO DITI'NS <br />FOR <br />HAUL ' OAD PERMIT <br />TRAFFIC CONTROL: <br />TRUCKS HAULING signs shall be appropriately placed along CSAH 54 (20th Ave.) in advance of <br />the haul access to CSAH 54. TRUCKS HAULING signs shall be removed from the roadway and <br />stored in such a manner as not to be visible to traffic when hauling operations are not in <br />progress. A STOP sign shall be placed at the haul road access to CSAH 54 as they exit the <br />site. If any incidents are reported or observed where contractor vehicles fail to stop and yield <br />to traffic on CSAH 54 hauling operations shall be terminated by ACHD and a flagging <br />operation will be required for the duration of the project. <br />If a flagging operation is implemented all signs and procedures used shall be consistent with <br />those given in the TEMPORARY TRAFFIC CONTROL ZONE LAYOUTS FIELD MANUAL dated <br />January 2007. All signs associated with the flagging operations are to be removed from the <br />roadway and stored in such a manner as not to be visible to traffic when hauling operation are <br />not in progress. <br />CLEAN UP OPERATIONS: <br />The Permit Holder shall provide a 75 foot long gravel approach at the access to CSAH 54 to <br />minimize the tracking of grading materials from the site onto the public highway. Continuous <br />clean up operations are required. County highways are to be swept (and scrapped if needed) <br />at least once per day. Additional clean up operations may be required if conditions warrant. <br />Bare pavement and visible roadway striping must be maintained at all times. Clean up <br />operations are to continue at the end of each work day until the road is clear of all material. If <br />_repolis_orJncidents of build up of material occur, hauling operations shall be terminated by <br />ACHD until clean up is completed. Contractor will be required to keep on site, or have <br />available within one hour, any equipment needed to accomplish clean up operations. Such <br />equipment may include sweeper, scrapper or bobcat, and a water truck. Failure to do so shall <br />be cause for ACHD to order the necessary work to be done and billed to the Permit holder. <br />All loads entering the county highway system must meet legal axle weight. <br />Dust control measures are also required. Hauling operations will be terminated if extreme <br />conditions exist. <br />lo <br />The Permit Holder shall be responsible for obtaining and complying with any permits deemed <br />necessary from the City of Lino Lakes. <br />
