The Performance Review <br />The essential job functions of each employee are identified in his/her position description. These <br />functions serve as the basis of the performance review. Rating symbols are attached to each job <br />function. <br />Rating Symbols <br />Four symbols are used to make the assessments as follows: <br />E = Exceeds Expectations (Performance has been above reasonable expectations). <br />F = Fully Competent (Performance has attained a level of full competency). <br />N = Need Improvement (Performance has been below reasonable expectations). <br />U = Unsatisfactory (Performance is unsatisfactory). <br />Suggestions for Improvements / Commendation <br />Spaces are available after each task for suggestions on how improvements to performance of the <br />task might be made, or to acknowledge commendation. <br />Year in Review /Looking Ahead <br />Once the job functions have been rated, the employee and the supervisor will review the major <br />accomplishments of the year, as well as a discussion of goals for the upcoming year. <br />Tools / Training /Education <br />The employee and the supervisor should take this opportunity to discuss what educational <br />opportunities or equipment the employee requires in order to enhance job performance. <br />The Position Description <br />This is an opportunity to revisit the position description, and determine if all essential job <br />functions, and knowledge, sldlls and abilities are recognized. The City reserves the right to <br />establish and modify all position descriptions. <br />Signatures / Comments /Follow -up <br />The employee has the right to make comments regarding the performance evaluation prior to <br />signing this document. <br />If performance is identified as `needs improvement' or 'unsatisfactory', a follow -up date will be <br />noted. <br />• <br />• <br />• <br />
