12. At least one person shall stand by outside the confined space ready to give assistance <br />in case of emergency. <br />a. The stand -by person shall have approved respiratory equipment (SCBA 15 min. <br />air pac) available for immediate use. <br />b. The stand -by person may enter the space only after notifying an emergency <br />response team, having a stand -by person available and only after donning the <br />required personal protective equipment (SCBA). <br />c. Visual, voice or signal line communications shall be maintained between <br />individuals within the confined space and the stand -by person(s). <br />13. When an entry must be made through a top opening the following requirements will <br />apply: <br />a. The safety harness shall be of the type that suspends a person in the upright <br />position. <br />b. An approved hoisting device or other effective means must be used to lift workers <br />out of the space. <br />c. A confined space entry permit is required prior to entry. <br />14. Hot work is prohibited if the flammability level is 10% of lower explosive level <br />(LEL) or greater. <br />15. At least one person trained in first aid and cardiopulmonary resuscitation (CPR) shall <br />be immediately available whenever respirator equipment is required. <br />V. EMERGENCIES <br />If an emergency situation arises, the observer is not to enter the confined space except as <br />provided below. In all cases the observer must: <br />A. Request assistance immediately from fire department, police, and rescue squad or other <br />maintenance personnel via the radio system or telephone. <br />B. Return immediately to the confined space and continue ventilation until assistance <br />arrives. <br />C. Do not enter the space to attempt a rescue without all proper protective equipment <br />(including SCBA) and until an observer is present. <br />D. Attempt rescue using lifelines only from outside the confined space until properly <br />equipped assistance arrives. <br />VI. TRAINING <br />Employees are expected to become and remain familiar with the terms of this policy. To <br />assist employees in becoming familiar with the policy the employer will do the <br />following: <br />A. Annual training will be given to employees regarding confined space entry. <br />1. Provide copies of the "Confined Space Entry Checklist" to all affected employees. <br />2. A copy of the policy will be issued to each employee regarding confined space entry. <br />B. Periodically the supervisor will conduct work -site inspections to assist employee in <br />complying with the policy. <br />VII. COMPLIANCE <br />Non compliance of this policy may result in disciplinary action. <br />22 <br />
