12. At least one person shall stand by outside the confined space ready to give assistance in case of emergency. a. The stand -by person shall have approved respiratory equipment (SCBA 15 min. air pac) available for immediate use. b. The stand -by person may enter the space only after notifying an emergency response team, having a stand -by person available and only after donning the required personal protective equipment (SCBA). c. Visual, voice or signal line communications shall be maintained between individuals within the confined space and the stand -by person(s). 13. When an entry must be made through a top opening the following requirements will apply: a. The safety harness shall be of the type that suspends a person in the upright position. b. An approved hoisting device or other effective means must be used to lift workers out of the space. c. A confined space entry permit is required prior to entry. 14. Hot work is prohibited if the flammability level is 10% of lower explosive level (LEL) or greater. 15. At least one person trained in first aid and cardiopulmonary resuscitation (CPR) shall be immediately available whenever respirator equipment is required. V. EMERGENCIES If an emergency situation arises, the observer is not to enter the confined space except as provided below. In all cases the observer must: A. Request assistance immediately from fire department, police, and rescue squad or other maintenance personnel via the radio system or telephone. B. Return immediately to the confined space and continue ventilation until assistance arrives. C. Do not enter the space to attempt a rescue without all proper protective equipment (including SCBA) and until an observer is present. D. Attempt rescue using lifelines only from outside the confined space until properly equipped assistance arrives. VI. TRAINING Employees are expected to become and remain familiar with the terms of this policy. To assist employees in becoming familiar with the policy the employer will do the following: A. Annual training will be given to employees regarding confined space entry. 1. Provide copies of the "Confined Space Entry Checklist" to all affected employees. 2. A copy of the policy will be issued to each employee regarding confined space entry. B. Periodically the supervisor will conduct work -site inspections to assist employee in complying with the policy. VII. COMPLIANCE Non compliance of this policy may result in disciplinary action. 22
