EXCAVATIONS & TRENCHING <br />Find the location of all underground utilities by contacting Gopher State One Call at (651) 454- <br />0002 before digging. <br />Keep workers away from digging equipment and never allow workers in an excavation when <br />equipment is in use. <br />Keep workers from getting between equipment in use and other obstacles and machinery that can <br />cause crushing hazards. <br />Keep equipment and the excavated dirt (spoils pile) back 2 feet from the edge of the excavation. <br />Have a competent person conduct daily inspections and correct any hazards before workers enter <br />a trench or excavation. <br />Provide workers a way to get into and out of a trench or excavation. Ladders and ramps can be <br />used and must be within 25 feet of the worker. <br />For excavations and utility trenches over five feet deep, use shoring, shields (trench boxes), <br />benching, or slope back the sides. Unless soil analysis has been completed, the earth's slope <br />must be at least 1 l horizontal to 1 vertical. <br />Keep water out of trenches with a pump or drainage system, and inspect the area for soil <br />movement and potential cave -ins. <br />Keep drivers in the cab and workers away when dirt and other debris are being loaded into dump <br />trucks. Workers must never be allowed under any load and must stay clear of the back of <br />vehicles. <br />FALL PROTECTION <br />Fall protection will be used at six feet and higher on any fixed ladder for climbing on any <br />scaffolding structure that does not have safety rails. Fall protection will be provided by the use <br />of a leg and waist type harness along with a lanyard with safety locking hooks at each end of the <br />lanyard. The lanyard will be attached to the personnel's leg/waist harness and structure. <br />FORKLIFT OPERATION SAFETY POLICY <br />The purpose of the Forklift Safety Policy is to ensure that employees have been trained in the <br />proper and safe use of pallet forks. <br />A. Loads must not exceed the maximum lift capacity of the machine and forks being used. <br />B. Loads in excess of the machines maximum lift capacity must be unloaded by hand until <br />the load is less than the machines maximum lift capacity. <br />C. Loads should be carried close to the ground when transporting. <br />D. Under no circumstance should an employee ride on a machine, only the operator in the <br />operator's seat is allowed. <br />E. Employees must use the required safety equipment when operating the machine. <br />23 <br />
