(b) Involves the laying, repairing, replacing, or enlarging of an underground <br />utility, pipe or other facility, or the disturbance of road ditch, grass swale, or other open <br />channel for a distance of 500 feet or more; or <br />(c) Is a land disturbing activity, regardless of size, that the City determines is <br />likely to cause an adverse impact to an environmentally sensitive area or other property. <br />(2) Grading, Erosion and Sediment Control Plan Requirements - The plan shall be <br />prepared and signed by a duly licensed professional engineer in the state of Minnesota. <br />The following exhibits must accompany the permit application. 2 plan sets, full size (22 <br />inches by 34 inches); one plan set, reduced to maximum size of 11 inches by 17 inches. <br />Additional copies may be required in accordance with applicable zoning and subdivision <br />provisions of city code. All plan sets shall also be submitted electronically in a .dwg <br />format or as otherwise determined by the city engineer. The minimum requirements of <br />the ESC Plan shall be consistent with the most recent version of the NPDES Permit <br />requirements and include the following information: <br />(a) Project name and type (residential, commercial, industrial, road <br />construction, or other); <br />(b) Project location; <br />(c) County parcel identification number (legal description); <br />(d) Names and addresses of the record owner, developer, land surveyor, <br />engineer, designer of the plat, and any agents, contractors, and subcontractors who will <br />be responsible for project implementation; <br />(e) Estimated start date, time frames, and schedules for each construction <br />phase, and completion date; <br />(f) Copies of permits or permit applications required by any other <br />governmental entity or agencies including mitigation measures required as a result of any <br />review for the project (e.g., wetland mitigation, EAW, EIS, archaeology survey); <br />(g) An 8.5 by 11 inch United States Geological Survey (USGS) 7.5 minute <br />quad, aerial photo, or equivalent map indicating site boundaries and existing elevations, <br />Map shall identify all natural and artificial water features (e.g., stormwater ponds, drain <br />tiles) on site and within one-half mile of the project boundary, including, but not limited <br />to lakes, ponds, streams (including intermittent streams), wetlands, and ditches. <br />(h) Existing Conditions Map. An existing site map and calculations, drawn to <br />a legible scale and clearly labeled with a north arrow and date of preparation.
