The grant requires that these systems shall be monitored for one year thereafter and the Engineer shall prepare a One Year Evaluation Report. Based on this report, the Engineer must certify that the project is adequately treating sewage in accordance with the State Permit and that the project is operating as designed. The project will be evaluated against the following criteria: 1. That the project has been completed In accordance with approved plans and specifications and change orders. 2. That there wil I be sufficient funds available to provide for adequate operation and maintenance of the project, as provided for in the Wastewater Service Charge Ordinance and Sewer Service Charge System. 3. That there is a sufficient number of trained and capable operating/maintenance personnel available to provide adequate operation and maintenance of the project. 4. That the project is being operated and maintained in conformance with the approved operation and maintenance manual. 5. Evidence that the project is in compliance with the applicable SDS permit; specifically, evidence that the systems are adequately treating septic tank effluent. This will be accomplished in the following manner: A. Each owner wit I be contacted by means of a malted questionnaire soliciting any problems or questions they may have on their system. All questions or problems will be followed up with a telephone call or field visit where 'appropriate.
