The grant requires that these systems shall be monitored for one <br />year thereafter and the Engineer shall prepare a One Year Evaluation <br />Report. Based on this report, the Engineer must certify that the project <br />is adequately treating sewage in accordance with the State Permit and that <br />the project is operating as designed. <br />The project will be evaluated against the following criteria: <br />1. That the project has been completed In accordance with approved plans <br />and specifications and change orders. <br />2. That there wil I be sufficient funds available to provide for adequate <br />operation and maintenance of the project, as provided for in the <br />Wastewater Service Charge Ordinance and Sewer Service Charge System. <br />3. That there is a sufficient number of trained and capable <br />operating/maintenance personnel available to provide adequate <br />operation and maintenance of the project. <br />4. That the project is being operated and maintained in conformance with <br />the approved operation and maintenance manual. <br />5. Evidence that the project is in compliance with the applicable SDS <br />permit; specifically, evidence that the systems are adequately <br />treating septic tank effluent. This will be accomplished in the <br />following manner: <br />A. Each owner wit I be contacted by means of a malted questionnaire <br />soliciting any problems or questions they may have on their <br />system. All questions or problems will be followed up with a <br />telephone call or field visit where 'appropriate. <br />
