5. Evidence that the project is in compliance with the applicable <br />SDS permit; specifically, evidence that the systems are <br />adequately treating septic tank effluent. This will be <br />accomplished in the following manner: <br />a. Each owner will be contacted by means of a mailed <br />questionnaire soliclting any problems or questions they <br />may have on their system. All questions or problems will <br />be followed up with a telephone call or field visit where <br />appropriate. <br />Response. Attached is the information and questionnaire <br />which was sent to each of the 131 participants in the 201 <br />Program. Each owner (and tenant in the case of rental <br />property) received an as -built drawing, pump literature, <br />information on how a septic system works, notification <br />that all tanks must be pumped every two years, and a <br />"troubleshooting" guide. <br />Of the 131 questionnaires mailed out, we have received <br />only 14 responses to -date. Of the 14 responses, only one <br />required a follow up field visit. We will continue to <br />follow up on the questionnaires as they arrive. <br />b. Individual on -site <br />systems and <br />all individual off -site, <br />or <br />clustered systems, <br />will be inspected in the field. <br />At <br />that time, all <br />inspection <br />and access covers will <br />be <br />removed and the system <br />checked <br />for uneven distribution <br />of <br />effluent, differential settlement, <br />or other evidence <br />of <br />poor performance. <br />Observation <br />of ground surface wll I <br />be <br />made to determine <br />if excessive <br />surface wetness or breakout <br />has occurred. <br />Response. The field inspection of the system is still on- <br />going and will be until the middle of December. Most of <br />our time has been concentrated so far on the cluster <br />system. The following is our evaluation of the cluster <br />systems: <br />1) Old Village Remote A - Legion Avenue <br />The pump station has been operating at 23% of its <br />design capacity and the drainflei d is operating at <br />16% of its capacity. These percentages will <br />increase to about 28% and 20% respectively when Dan <br />Collyardts house is occupied. <br />2) Old Village Remote B - Lisbon Avenue Townhouses <br />Based on water meter readings, the system is <br />operating at about 18% of its design capacity. <br />3) Old Village Remote C - 32nd Street <br />Based on water meter readings, the system Is <br />operating at 23% of its design capacity. <br />-3- 8566 <br />
