The name and address of the subdivider, developer, and owner <br /> or any other party of interest. <br /> 2. A legal description of the proposed site with a map showing <br /> its location with indications of private access roads, <br /> existing or proposed public roadways within and surrounding <br /> the development site. <br /> 3. A full and adequate description of all phases of the operation <br /> and/or proposed physical changes. <br /> 4.
