Permits must be signed and returned to the Community Center to complete the reservation <br /> process. Permits define the times, fees,cancellation provisions and conditions related to the <br /> • facility use. <br /> Liability <br /> The applicant and/or organization agrees to assume all responsibility for damage or liability of <br /> any kind and further agrees to hold the City of Mounds View harmless from any expenses or <br /> costs in connection with use of the community center facilities. The Community Center may <br /> require that applicant and/or organization to furnish a certificate of insurance to guarantee the <br /> conditions of the facility use agreement or any liability incurred by it. <br /> Cancellations <br /> The user accepts responsibility for costs and liabilities should the event be canceled due to <br /> scheduling conflicts or inclement weather. A refund of any rental already paid will be made if the <br /> event cannot be rescheduled. Cancellations should be made at least 48 hours prior to rental date. <br /> Supervision <br /> The group, individual, or organization using the Community Center must provide competent adult <br /> supervision for the entire time participants are in the building or on surrounding grounds. <br /> Damage Deposit <br /> It is understood that any and all costs incurred by the City of Mounds View and the Mounds View <br /> Community Center for cleaning or repairs will be paid for out of the required damage deposit <br /> ($100). If the damage exceeds$100,the additional cost incurred will also be the responsibility of <br /> • the renter. Failure to properly clean the room will result in the forfeiture of the total amount of <br /> the damage deposit. Deposits will be mailed back to the renter within one week of the rental date. <br /> Cleaning supplies include: vacuums,paper towels,and cleaning solution,which are all <br /> located at the facility front desk. <br /> General Rules <br /> 1. Residents may not reserve the facility for non-residents <br /> 2. Reservations may not be made more than 12 months in advance by any group <br /> 3. If payment is required,this must be made at least two days prior to the scheduled <br /> reservation date. Rooms are not considered reserved until payment is received. <br /> 4. Weights for helium filled balloons cannot be filled with birdseed,rice, sand, or <br /> similar items. Suggested items to use as weights are small bags with candy,mints, <br /> metal washers, stone or brick. <br /> 5. Decorations may not be attached to any walls, carpeting, flooring, or the ceiling in <br /> any form. Ceiling hooks can be provided by the facility. Please request in advance. <br /> 6. Decorations may not be hung from sprinkler heads (fire code regulation) <br /> 7. Decorations may be taped to the underside of tables using only masking tape. <br /> Absolutely no other tape is allowed (i.e. duct tape,scotch tape,book binding, <br /> etc) <br /> 8. Rooms should be left with the same arrangement as upon arrival. Unless specific <br /> arrangements have been made,room set-up will not be made available by the <br /> Community Center staff Room set-up requests are the responsibility of the renter <br /> and need to be approved by the Community Center Director. <br /> 9. Trash should be placed in designated containers. <br /> 10. Organized activities to conclude at the closing time stated on the permit. <br /> • <br />
