RESOLUTION NO. 8187 CITY OF MOUNDS VIEW COUNTY OF RAMSEY STATE OF MINNESOTA RESOLUTION APPROVING SEVERANCE PAYMENT TO STEVE DAZENSKI, PARKS SUPERVISOR WHEREAS, Steve Dazenski submitted his resignation for retirement and his last day of employment was December 4, 2013; and WHEREAS, the balance of accrued and unused sick leave time is 610.36 hours; and WHEREAS, the balance of accrued and unused vacation is 124.16 hours; and WHEREAS, Steve Dazenski's current rate of pay is $31.28 per hour; and WHEREAS, in accordance with Section 3.45 of the Mounds View Personnel Manual, Mr. Dazenski is qualified to contribute 1,280 sick hours to his RHSP; and WHEREAS, from 2008 to present, Mr. Dazenski has contributed 794.25 sick hours to his RHSP; and WHEREAS, due to the base cap on sick hours contributed to the RHSP, Mr. Dazenski is allowed to contribute a maximum of 485.75 sick hours into his plan; and WHEREAS, in accordance with Section 3.45 of the Mounds View Personnel Manual, the maximum potential payout of accrued/unused sick time will be 485.75 hours x 65% x his hourly rate of $31.28 = $9,876.27 into Mr. Dazenski's Retirement Health Savings Plan; and WHEREAS, in accordance with Section 3.47 of the Mounds View Personnel Manual, the maximum cash potential payout of accrued/unused vacation time will be 124.16 hours x his hourly rate of $31.28 = $3,883.72. NOW, THEREFORE, BE IT RESOLVED that the Mounds View City Council approves a one-time payout of sick time not to exceed $9,876.27 into Steve Dazenski's Retirement Health Savings Plan in accordance with Section 3.45 of the Mounds View Personnel Manual. NOW, BE IT FURTHER RESOLVED that the Mounds View City Council approves a one- time cash payout of vacation time not to exceed $3,883.72 to Steve Dazenski in accordance with Section 3.47 of the Mounds View Personnel Manual.
