1. Payment of severance pay in the amount of one week of the employee's <br /> annual salary for every year of the employee's service to the City; and <br /> 2. Payment of unused vacation and sick leave in an amount equal to the full <br /> amount available of the employee's unused vacation leave at the employee's time of <br /> departure from City service, and 50% of the employee's amount of accumulated sick <br /> leave at the employee's time of departure from the City; and <br /> 3. Payment by the City of an amount equivalent to six months of the City's <br /> standard contribution normally paid for an employee's health insurance premiums; and <br /> 4. Payment by the City, up to a value of $3,000, for outplacement <br /> employment services, with such payment to be made to an outplacement services <br /> company as agreed upon by the City and employee; and <br /> 5. That the offering of the above severance benefits by the City is subject to <br /> the execution of a release of all claims by the employee against the City and its agents, <br /> consultants, employees, former employees, insurers, heirs, executors and assigns, in such <br /> language as is required and approved by the City. <br /> NOW, THEREFORE, BE IT FURTHER RESOLVED THAT the City Council directs <br /> City staff to prepare the appropriate documents and agreements to facilitate the offering and <br /> execution of the severance plan/program as provided herein. <br /> Adopted this_day of December, 1998 <br /> Mayor Duane McCarty <br /> City-Clerk Administrator, Charles S.
