We are very excited about possibility of having a center in 612/646-7771 PHONE <br /> the Bel Rae complex. 612/646-8676 FAX <br /> In response to your request we are forwarding the following list of items, we feel, need to <br /> be incorporated into the lease arrangement between Children's Home Society and the <br /> City of Mounds View. <br /> 1. Set up the lease as a final and add a clause addressing the interim period. By41111 <br /> including a section for converting would limit exposure to any sudden changes once the <br /> building remodeling is done. <br /> 2. Approval of any modification to the original design, CHSM specification, for the <br /> remodeling and interim space used by CHSM. Requiring written notification, time limits, <br /> and liability for changes made without CHSM's approval. <br /> 3. We need a section in the lease addressing the addition of other services beyond <br /> the initial start up phase, for example infant care program, 24 hour care, etc. <br /> 4. Clarify liability for any injuries which may happen in the common area or outside <br /> the building, for example the playground area or park area. <br /> 5. Establish a rent figure which would be two tiered, base rate plus an amount tied <br /> to the occupancy of the child care center. (See example) <br /> 6. Included in the lease rate maintenance, cleaning, snow removal, garbage <br /> removal, etc. <br /> The intent here is to get away from subcontracting for these services. <br /> 7. Clarify that this is a separate entity and may operate at different hours then the <br /> rest of the building. <br /> 8. Food service, we would like to operate the food service and will be open to <br /> working out cost with others that may be interest in food service. <br /> 9. Signage for the center, type and limitations. We will need a large sign on the <br /> highway and another one on the building. <br /> 10. Phones for the center separate or part of any internal system. • <br />
