Item No: 7H <br />Meeting Date: December 12, 2016 <br />Type of Business: Council Business <br />Administrator Review: ____ <br /> <br />City of Mounds View Staff Report To: Honorable Mayor and City Council <br />From: Brian Erickson, Public Works Director <br />Item Title/Subject: Resolution 8680, Accept Improvements and Approve Final <br />Payment for 2015 Street and Utility Improvement Project – Area H <br /> <br />Introduction: <br />The City Council is asked to approve the attached resolution accepting the improvements and <br />authorizing final payment for the 2015 Street and Utility Improvement Project – Area H (City <br />Project No. 2014-C04). <br /> <br />Discussion: <br />The City of Mounds View is in the process of reconstructing 26 miles of City-owned streets and <br />associated underground utilities as part of the Streets and Utility Improvement Program <br />(Program). The Program includes design standards, a financing plan, a <br />prioritization/implementation (staging) plan, and a communication plan for implementing <br />reconstruction over a 9-year period (2009 – 2018) with an estimated program cost of $28 <br />million. One of the projects approved by City Council was the 2015 Street and Utility <br />Improvement Project – Area H (City Project No. 2014-C04) (the Project). <br /> <br />City Council has taken the following steps in completing this project: <br />• July 14, 2014, Approve Resolution 8281 Authorizing Stantec Consulting, Inc. to Perform <br />Professional Engineering Design Services and Prepare Bidding Documents for Area H <br />of the Street and Utility Improvement Program. <br />• November 24, 2014, Approve Resolution 8327 Considering Stormwater Infiltration <br />Program (SIP) Appeals in Area H of the Street and Utility Improvement Program. <br />• November 24, 2014, Approve Resolution 8328 Approving a Non-Standard Street <br />Design and Parking Restrictions for Ardan Avenue between Spring Lake Road and Red <br />Oak Drive. <br />• November 24, 2014, Approve Resolution 8329 Approving a Non-Standard Street <br />Design and Parking Restrictions for Groveland Road between County Road 10 and <br />County Road J. <br />• December 8, 2014, Approve Resolution 8345 Authorizing Stantec Consulting, Inc. to <br />Perform Professional Engineering Design Services and Prepare Bidding Documents for <br />Groveland Lift Station Rehabilitation. <br />• February 23, 2015, Approve Resolution 8377 Approving Plans and Specifications for <br />2015 SUIP – Area H. <br />• April 13, 2015, Approve Resolution 8393, Awarding a Construction Contract to <br />Northwest Asphalt, Inc. for the 2015 Street and Utility Improvement Project – Area H. <br />• November 9, 2015, Approve Resolution 8477 Authorize Stantec to Perform <br />Construction Services for the Groveland Lift Station. <br /> <br />Northwest Asphalt, Inc. submitted the lowest responsible bid and was awarded the construction <br />contract for the Project. Northwest Asphalt performed most of the the construction in the <br />summer of 2015 and completed their work in the late spring of 2016. They were substantially <br />complete on May 27, 2016. The punch list that was prepared following construction has been <br />completed and as a result Northwest has met nearly all the requirements of the contract <br />documents. They are currently collecting all final paperwork and releases, which will be verified <br />by Stantec and Mounds View staff prior to releasing final payment. The project quantities were <br />finalized on December 5, 2016, and Stantec has been waiting to receive final paperwork from
