To complete your PATROL <br />renewal, this form must be completed, signed by the Chief Law Enforcement Officer, and returned to the <br />League of Minnesota Cities Insurance Trust (LMCIT) by email (patrol@lmc.org) or fax (651-215-4155). <br /> <br />If you have any questions about processing your PATROL renewal, please contact Helene Tetz at <br />patrol@lmc.org or 651-215-4095. <br /> <br />Subscription Agreement <br /> <br />I. This subscription agreement is entered into by the League of Minnesota Cities Insurance Trust <br />and the City of Mounds View for the period of January 1, 2016 through December 31, 2016. <br /> <br />II. I, Jim Ericson, City Administrator and Joe Flaherty, Mayor, am authorized to enter into this <br />agreement on behalf of the City of Mounds View. I can be reached by phone at 763-717-4001 or <br />email at jim.ericson@ci.mounds-view.mn.us. <br /> <br />III. I am purchasing subscription to PATROL for 19 law enforcement officers. I understand the <br />subscription I purchase allows named law enforcement officers to participate in PATROL <br />training. <br /> <br />IV. My city or county is (check one) <br />x A member of the League of Minnesota Cities Insurance Trust (LMCIT) or the Minnesota <br />Counties Intergovernmental Trust (MCIT) (subscription rate is $85 per officer per year). <br /> A member of the League of Minnesota Cities (LMC) or the Association of Minnesota <br />Counties (AMC) but not a member of LMCIT or MCIT (subscription rate is $95 per <br />officer per year). <br /> Not a member of the LMCIT, MCIT, LMC, or AMC (subscription rate is $110 per officer <br />per year). <br /> <br />V. I understand I will be invoiced an annual fee per student, prorated monthly to reflect my agency’s <br />enrollment. <br /> <br />Please send the invoice to the attention of Chief Thomas Kinney, Mounds View Police <br />Department, 2401 County Road 10, Mounds View, MN 55112. <br /> <br />VI.
