11 tit <br />December -January4 <br />Date of Report: February 7, 2014 <br />Report Submitted to: Bethlehem Baptist Church <br />Bethlehem Baptist contributions helped fund the employment of a Community Social <br />Worker (CS99 to coordinate services at the Community Support Center; a faith, service, <br />school collaboration at Pike Lake Ed Center, in order to prevent and divert homelessness <br />and housing instability. <br />Report Narrative <br />Through collaborative partner referrals and Ministry Teams and on-site supportive services, we <br />anticipated serving about 300— 480* individuals/families In a year, thus intervening or <br />preventing homelessness. We also anticipated hosting group classes (approximately 240 people <br />a year). Our tag line is "Neighbors helping neighbors and partners helping partners. " This <br />means that our face to face services with neighbors and our phone and collateral supports with <br />partners all are included in `numbers served'. <br />From December, 2012 through January, 2014: 172 <br />REFERRALS/NUMBERS SERVED: <br />Face to face (163) <br />Phone (304) <br />• Collateral referrals (350) <br />TOTAL referrals/numbers served*: 817 (anticipated 300 — 480). We hit our target in this <br />area. <br />*The total UNDUPLICATED count of individuals/families served in this time period is 172. <br />*duplicated count. <br />CLASSES: <br />• Parenting Class 5/29113 (12 attendees) all female, 3 African American, 2 Latino and 7 <br />Caucasian; ages 18 -30 (10); ages 30 — 42 (2) <br />• Financial Management Class 4122/13 (0 attendees) <br />• Anxiety/Depression Class (6/28113) (0 attendees) <br />*We had anticipated 20 participants attending classes monthly. We fell very short on this <br />outcome. <br />ALLYSUPPORT: <br />• 4 Ally partnerships established (1 est. in Feb, 2013; white, single male); (I est. in Aug, <br />2013; Southeast Asian single father with 3 children); (I est. in Nov 2013; African <br />American single mother with 4 children); (1 est. in Jan. 2014; Latina single mother with <br />6 children) <br />*We anticipated making 2 ally matches a month and again fell short on this outcome. <br />What progress have you made toward your original goals and objectives? <br />
