The TOTAL referrals/numbers served*: 817 (anticipated 300 — 480). We hit our target in <br />this area. *The total UNDUPLICATED count of individuals/families served in this time <br />period is 172. We hit our target in this area. <br />Because we were able to use the CSW position to serve our neighbors readily at the CSC, <br />interest in the collective work began to grow and partners remained engaged throughout <br />the year, despite our ability to mobilize the volunteer roles and responsibilities of the <br />center. Engaging and growing our relationships with the faith community was a key <br />success in Year I's pilot. One example of our growth in the partnership development is <br />the fact that we now have engaged our Senior Pastors in discussions about financially <br />sustaining the operations of the CSC through `Covenant Partnerships' that are multi- <br />yearfocused <br />What activities led to meeting these goals and objectives? <br />® One on one, face to face, phone interactions with neighbors via the CSW. <br />® One on one, face to face, phone interactions with CSW and partners where coordination <br />of services occurred through the CSW which helped build trust among partners and <br />facilitated positive outcomes with neighbors. This work proved to partners that a <br />collaborative effort to support neighbors experiencing complex barriers and housing <br />instability is afar better and more respectful approach in transforming situations from <br />crisis to stability. <br />® Speaking engagements directed at connecting with our faith partners. The focus was on <br />the value of capacity building, teaming and service coordination vs. referring and <br />`handing off a family/individual to the CSC. <br />® Monthly multi -sector team meetings and monthly advisory team meetings were held. <br />® A logo was created that illustrated a `huddle' of all working together to find solutions <br />that strengthen housing stability in the community. With this logo, came the tag line: <br />Neighbors Helping Neighbors; Partners Supporting Partners. We're trying to get away <br />from this being just one more program in the sea ofprograms; and make clear that this <br />is a multi -sector collaboration that is strengths focused and community based. <br />® The work we did to roll out the CSC at church picnics, blockparties, and social <br />concerns meetings and social justice leadership groups has helped solidify our model in <br />the community. The communiques and calling cards (vs. a brochure) helped get the CSC <br />information into the hands of ordinary community members not involved directly with <br />the formation of the CSC. These calling cards will continue to help our community <br />`own' this work and mobilize this effort as community based <br />® Including senior pastors in our dialogue has been an incredible lift to the work of the <br />CSC. <br />The population served or community reached during the grant period: <br />Demographics of neighbors served in the timeframe December 2012 — January 2014: <br />o Total served: 172 <br />o Male: 34 <br />o Female: 138 <br />® White: 72 <br />® Afi^ican Americana 65 <br />® Native American: 1 <br />® Latino: 11 <br />® SEAsian:2 <br />
