exact date of the birth, adoption or placement is unforeseeable, the employee must provide <br /> • <br /> notice as soon as practicable. <br /> POLICY: MISCELLANEOUS PAID/UNPAID LEAVES <br /> Approved By: Federal, State, City Code Section: 2.05 <br /> Revision History: 1992, 1997 <br /> A. Blood Donation <br /> As the City recognizes the societal benefits derived from giving blood, employees may <br /> take up to two hours give blood every seven weeks. This time limit may be extended in <br /> certain circumstances (i.e., rare blood type). Employees must request time off from <br /> their respective supervisor in advance. <br /> B. Bone Marrow Donation Leave (MN §181.945) <br /> Employees who work an average of 20 or more hours per week may take up to 40 paid <br /> hours to undergo a medical procedure to donate bone marrow. Verification by a <br /> physician of the purpose and length of leave is required. If there is a medical <br /> determination that the employee does not qualify as a donor, the paid leave taken prior <br /> to the medical determination is not forfeited. Vacation, sick leave and holiday time will <br /> accrue during the leave. <br /> C. Emergency Leave (Clerk Administrator) • <br /> In cases of serious emergency, including death or serious illness in the employee's <br /> family, with family as defined in Funeral Leave Policy, or other emergency of a serious <br /> nature, a regular employee's Department Head may authorize use of sick leave benefits <br /> for such absences up to a maximum of three days for each emergency. The number of <br /> emergency leaves per employee shall be determined on a case-by-case basis by the <br /> City Administrator. <br /> D. Funeral Leave (State) <br /> A maximum of up to three (3) days off with pay will be allowed following the death of a <br /> family member, defined as a spouse, son, daughter, father, brother, sister, grandmother, <br /> grandfather, father-in-law, mother-in-law, sister-in-law, brother-in-law, daughter-in-law or <br /> grandchild. Regular part-time employees will be granted three (3) half days funeral <br /> leave, as requested. Additional days may be approved by the Department Head, taken <br /> as needed and charged to accrued sick leave and/or vacation. If sick time and/or <br /> vacation time is not available, the additional days may be taken as time off without pay. <br /> The number of additional days taken off with or without pay will be determined on a <br /> case-by-case basis by the City Administrator. <br /> Time off for the funeral leave of any other relative or friend may be granted by the <br /> Department Head, but such time off shall not be eligible for funeral leave pay and shall <br /> be charged to vacation leave or taken as time off without pay. <br /> E. Jury Duty (State) <br /> 41 <br /> Any regular or probationary employee who is required to serve as a juror or who is <br /> under subpoena as a witness in court on City matters, will be granted leave with pay <br />
