not accrue during an unpaid leave of absence. <br /> • <br /> Step increases and probationary periods will be extended by the length of the unpaid <br /> leave. The newly established anniversary date will be used for all future pay increases, <br /> however the employees hire date will not change because of such leave. <br /> Insurance Continuation <br /> During unpaid leave, the employee may continue to be covered by group health, dental <br /> and life insurance, but will be responsible for paying one hundred percent (100%) of the <br /> premium costs. During the first calendar month the employee will be responsible for <br /> paying the pro-rated portion of their monthly insurance premiums for the days they are <br /> on unpaid leave. <br /> The employee must return to work the first day following the end of the leave and must <br /> notify their Department Head or City Administrator at least one (1) month prior to that <br /> date of their intention to return. <br /> L. Job Related Leave (Clerk Administrator) <br /> A job related leave policy has been established to grant employees an extended period <br /> of time off without pay for job related reasons. A regular full-time employee may request <br /> a period of time off without pay for a period of not less than three or more than six <br /> months. The employee must substantiate the request for such leave by relating it to the <br /> exploration of career change, to educational advancement which would enhance <br /> present position performance, or to stress factors associated with the employee's <br /> present situation. <br /> Procedures for approval and policies regarding status of benefits and continuation of <br /> insurance stated under"Unpaid Leave of Absence" shall also apply to related Job <br /> unpaid Leave of Absences. <br /> POLICY: WORKING HOURS, ATTENDANCE, and BREAKS <br /> Approved By: City Code Section: 2.10 <br /> Revision History 1988, 1997 <br /> Working Hours and Attendance <br /> Each employee is vital to the operation of the City and therefore each employee must comply <br /> with City rules and department rules. Employees are expected to work the normal hours and <br /> the minimum number of hours per week as established for the position. The City expects each <br /> employee to come to work on time, and regularly. From time to time, absences and tardiness <br /> are unavoidable. If an employee is must be absent or late, they are expected to notify their. <br /> Supervisor, or the person designated by their Supervisor, as soon as possible. Excessiv,c <br /> tardiness or absence may result in disciplinary action, including discharge. <br /> Each employee shall ensure that time worked and absences are correctly reported and the <br /> . distribution of hours worked are charged to their proper accounts. Supervisors are also <br /> responsible for proper time sheet entries. <br />
