APPLICATION INSTRUCTIONS <br /> 1. Read the application packet and call the Board's staff at 612/282-2390 if you have any <br /> questions or need more information. <br /> 2. Make a note of the grant number and grant category assigned to the project by the Board <br /> in its notice informing you of the score you received on your pre-application. Contact the <br /> Board's staff if there are any concerns about the grant category assigned by the Board. <br /> 3. Convene the potential partners and discuss everyone's ideas. It is important to have <br /> everyone involved in developing the application. <br /> 4. In collaboration with all potential partners: <br /> a. Refine the proposal in accordance with the application forms. Be sure that all the <br /> issues in the APPLICATION NARRATIVE FORMAT are discussed and agreed upon, <br /> including a work product and a planning or implementation process. <br /> b. Determine which potential partners are in sufficient support of the project to be <br /> included in the application. Please note that the Board will consider all applicants <br /> included on the application to have committed to participating in the project. <br /> c. Develop a detailed budget for the project and determine how much money will be <br /> requested from the Board. This amount may differ from the pre-application, but <br /> may not exceed the statutory maximum for the application's grant category. <br /> d. Determine who the contact person will be for the application: This person should <br /> be able to answer questions about the application. <br /> 5. For all partners that will be included in the application, determine which have exclusive <br /> representatives (unions) representing employees who provide the program or service that <br /> will be affected by the application. Note that a copy of the application will have to be sent <br /> to each of these exclusive representatives. <br /> 6. Prepare the APPLICATION COVER PAGE and NARRATIVE. Use the forms for the correct <br /> grant category. Answer all the questions on the COVER PAGE and be sure that all the issues <br /> in the NARRATIVE FORMAT have been addressed. <br /> 7. Obtain all necessary signatures. Note that the application must be signed by the senior <br /> elected or administrative official of each applicant. However, the signatures do not have <br /> to be originals (facsimile copies are acceptable) and each applicant may sign on a different <br /> sheet of paper if necessary. <br /> 8. Use the "Checklist for Mailing Your Application" to be sure all requirements are met. <br /> 9. Mail or deliver the application packet. Please do not fax applications to the Board's <br /> Office. Be sure the Board will receive it by 5:00 p.m. on March 15, 1996. Applications <br /> received after the deadline will not be considered. <br />
