Explanation/Summary (attach supplement sheets as necessary.) KMMARY; At the February 6th Work Session, representatives from Lake Region Hockey, requested that the Council allow them to conduct charitable gambling at Donatelle's Restaurant. It was noted by Lake Region Hockey that their organization's address was not in Mounds View as required by the Mounds View Municipal Code. Lake Region Hockey provided a copy of the minutes of their February 28, 1995 meeting minutes at which the organizations 's registered office was moved to the City of Mounds View. As you requested, I contacted Lake Region Hockey and advised them that they would need to supply the City of Mounds View the following items by March 15 to be included in the packet for March 20th: a. Completed State Application Form b. Requested from Lake Region's 1994 audit the percentage and dollar amounts given to the City of New Brighton. c. Also, requested a copy of the form sent to the State showing the charitable gambling contributions to the City, school district and other non-profit organizations . Lake Region Hockey delivered the documents to the City offices on the afternoon of March 15th, however, the State application was not complete in that there was no lease agreement and the floor plan was incomplete.
