It was noted by Lake Region Hockey <br /> that their organization's address was not in Mounds View as required by <br /> the Mounds View Municipal Code. <br /> Lake Region Hockey provided a copy of the minutes of their February 28, <br /> 1995 meeting minutes at which the organizations ' s registered office was <br /> moved to the City of Mounds View. <br /> As you requested, I contacted Lake Region Hockey and advised them that <br /> they would need to supply the City of Mounds View the following items by <br /> March 15 to be included in the packet for March 20th: <br /> a. Completed State Application Form <br /> b. Requested from Lake Region' s 1994 audit the percentage <br /> and dollar amounts given to the City of New Brighton. <br /> c. Also, requested a copy of the form sent to the State <br /> showing the charitable gambling contributions to the City, <br /> school district and other non-profit organizations . <br /> Lake Region Hockey delivered the documents to the City offices on the <br /> afternoon of March 15th, however, the State application was not complete <br /> in that there was no lease agreement and the floor plan was incomplete. <br /> The item was not put on the agenda because of the incompleteness of the <br /> documentation. <br /> , /, / i <br /> Michele Severs• • , Administrative Secretary/Deputy Clerk <br /> RECOMMENDATION; <br />
