D a Concrete <br /> iWiLa Surfacing, Inc. <br /> P.O. Box 185 • Rogers, Minnesota 55374 • (612)535-2644 • Fax (612)535-6934 <br /> PREPARATION <br /> A. The purpose of preparation is to open the surface creating a clean and dry profile in order to provide <br /> maximum adhesion/penetration of the coating system. <br /> B. Surface preparation is best achieved through the use of a steel shot blasting machine. It is a self-propelled <br /> and self-contained unit in which 95%of the steel shot is recovered and reused. Contaminates such as dirt, <br /> oil,coatings,spaulding,etc.are removed thereby creating a clean open surface. The unit also vacuums and <br /> retains all dirt, dust and debris from the removal making the process virtually dust free. <br /> CONDITIONS <br /> C. The area to be prepared for coating will not be walked on by anyone other than the floor preparer from the <br /> commencement of the job until the proper cure time has elapsed after the last application. This is very <br /> important and must be strictly enforced in order for the warranty to be valid. <br /> D. Building owner shall provide an electrician and power source:208/230 volt.3-phase.60 ami service <br /> to be connected 1/2 Hour prior to start of project and disconnect on completion. <br /> E. The building owner shall provide a dumpster suitable for the holding of removed floor materials and waste <br /> as well as empty product containers and other debris. Disposal of such items shall be the responsibility of <br /> the building owner, in accordance with federal, state, and/or local regulations. <br /> EXCEPTIONS <br /> F. This proposal does not include the moving of equipment,supplies,or other personal property from the area <br /> to be prepared or coated,nor the removal of paint overspray,concrete spillage,or stains,unless otherwise <br /> specifically stated in the work order. <br /> G. There may be products,ingredients or personnel in the facility that are subject to the absorption or inhalation <br /> of solvent odors. If so, it will be the responsibility of the owner or his agent to protect such products, <br /> ingredients or personnel from the solvents in the materials used in preparation and coating of the floor. <br /> H. There may be equipment,products or personnel in the facility that are subject to damage from dust or shot. <br /> If so,it will be the responsibility of the owner or his agent to protect such equipment,products or personnel <br /> from any dust or shot generated while preparing the floor for coating. <br /> WARRANTY <br /> *** P.S.I. Concrete Surfacing, Inc. warrants that all coatings and resurfacers applied pursuant to this <br /> agreement will not peel due to improper preparation and application for one year. This application <br /> warranty does not include damage to the coating from substrate failure,hydrostatic pressure,severe <br /> or abnormal use. This warranty and is in lieu of all other rights and remedies. <br />
