Item No: 08B <br />Meeting Date: January 22, 2007 <br />Type of Business: CA <br />City Administrator Review: _____ <br />City of Mounds View Staff Report <br />To: Honorable Mayor and City Council <br />From: Desaree Crane, Assistant to the City Administrator <br /> <br />Item Title/Subject: Resolution 7005 Approving a Therapeutic Massage <br />Business License to Audrey Olsen, who will be <br />administrating Therapeutic Massage at the Silverview <br />Chiropractic Center in Mounds View <br /> <br />Audrey Olsen is applying for a Therapeutic Massage License to practice therapeutic <br />massage at the Silverview Chiropractic Center in Mounds View. Ms. Olsen will be <br />assisting Dr. Greg Belting (Chiropractor with Silverview Chiropractic Clinic) in massage <br />therapy care. She currently works in the business office for Dr. Belting and has been <br />employed with Silverview Chiropractic Clinic for 12 years. <br /> <br />Ms. Olsen has the necessary insurance coverage in accordance with the City Code, <br />received her training through the Minneapolis School of Massage and Bodywork located <br />in Minneapolis, and completed 587.50 hours of training (400 hours is required in <br />accordance with the Mounds View City Code). In addition, Ms. Olsen has been practicing <br />massage therapy on family, friends and the public since beginning the massage therapy <br />program at the Minneapolis School of Massage and Bodywork on May 13, 2003. (City <br />Code requires that 1 year of practicing massage therapy as required by affidavit – <br />Affidavit is on file with the City). All applicable license fees have been paid in full. <br /> <br />The City Code also requires a background check on Ms. Olsen. Staff has not received <br />the background check back from the Minnesota Bureau of Criminal Apprehension. This <br />Therapeutic Massage License will be pending a satisfactory background check. <br /> <br />In accordance with the City Code, this license will expire on December 31, 2007. <br /> <br />Recommendation: <br />Staff recommends approval of Resolution 7005, approving Ms. Olsen’s Therapeutic <br />Massage license pending a satisfactory background check. <br /> <br />Respectfully submitted, <br />Desaree Crane <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />
