RESOLUTION NO. 7005 <br /> <br />CITY OF MOUNDS VIEW <br />COUNTY OR RAMSEY <br />STATE OF MINNESOTA <br /> <br />APPROVING A THERAPEUTIC MASSAGE LICENSE <br />TO AUDREY OLSEN, SILVERVIEW CHIROPRACTIC CLINIC IN MOUNDS VIEW <br /> <br /> <br />WHEREAS, the City Code requires that massage therapist must be licensed in the City; <br />and <br /> <br />WHEREAS, the City Code requires that the applicant have one year of experience <br />practicing massage therapy as established by an affidavit and can document within two years of <br />obtaining the license that the applicant has completed four hundred hours or certified <br />therapeutic massage training from a recognized school; and <br /> <br />WHEREAS, Audrey Olsen is applying for a Therapeutic Massage License to practice <br />therapeutic massage at the Silverview Chiropractic Center in Mounds View; and <br /> <br />WHEREAS, Ms. Olsen has the necessary insurance coverage in accordance with the <br />City Code; and <br /> <br />WHEREAS, Ms. Olsen received her training through the Minneapolis School of Massage <br />and Bodywork located in Minneapolis, and completed 587.50 hours of training; and <br /> <br />WHEREAS, Ms. Olsen submitted an affidavit which stated that she has provided <br />therapy to family friends and the public since beginning the massage therapy program at the <br />Minneapolis School of Massage and Bodywork; and <br /> <br />WHEREAS, it is required that the applicant must have a satisfactory background check; <br />and <br /> <br />WHEREAS, this Therapeutic Massage License will not be awarded unless staff <br />receives a satisfactory background check from the Minnesota Bureau of Criminal Apprehension. <br /> <br />NOW, THEREFORE BE IT RESOLVED, a Therapeutic Massage License is approved to <br />Audrey Olsen, pending a satisfactory background check. <br /> <br />NOW, MAY IT FURTHER BE RESOLVED, that Ms. Olsen’s Therapeutic Massage <br />license will expire on December 31, 2007. <br /> <br />Adopted this 22nd day of January, 2007. <br /> <br /> <br />___________________________________ <br />Rob Marty, Mayor <br /> <br />ATTEST: <br /> <br />___________________________________ <br />Kurt Ulrich, City Administrator <br /> <br />(seal)
