The award selection committee (City Council) will identify one exceptional <br />individual who meets the following criteria: <br /> <br />1. Courage: Exemplifies courage and determination to follow one’s own <br />values. <br /> <br />2. Initiative: Demonstrates drive and initiative in his/her approach and <br />overcoming challenges along the way. <br /> <br />3. Time Commitment: Volunteers a significant amount of time and energy to <br />further a cause or issue; more than just financial contributions. <br /> <br />4. Effectiveness: Generates positive results for the community. <br /> <br />5. Unselfishness: Does not expect anything in return for his/her actions, i.e. <br />financial compensation, fame, or political notoriety. <br /> <br /> <br />How to Nominate <br /> <br />Nominations must be in writing and may be supported by letters and other <br />documentation. All materials must be received by the close of the nomination <br />period, and become the property of the City of Mounds View. <br /> <br />The deadline for nominations for 2004 is Friday, October 15, 2004. <br /> <br />Complete the nomination form/Gather letters and documents to support the <br />nomination. <br />Send all nomination materials to: <br />The City of Mounds View Citizenship Award <br />2401 County Highway 10 <br />Mounds View, MN 55112 <br />Ph: (763) 717-4000; Fax: (763) 784-3462 <br /> <br />Other Considerations for Council <br /> <br />Budget – the City would have enough in the City Council Employee Recognition budget <br />this year to fund an engraved plaque for the recipient of the Citizenship Award (est. $50 <br />to $75). This plaque could be presented at a Council meeting. This plaque could be <br />presented at a Council meeting on November 8th, upon review and selection of a <br />candidate at the October 18th Work Session. <br /> <br />Recognition of Other Retiring Commission and Board members will continue to be done <br />with a signed and framed resolution presented at Council meetings. <br /> <br />Recommendation: <br />It is recommended that the City Council approved the attached Resolution 6361, <br />approving the Mounds View Citizenship Award policy <br /> <br /> <br /> <br /> <br /> <br />
