<br />Permits Required <br /> <br />The contractor shall, at the contractor’s own expense, obtain all required licenses and permits in <br />relation to the demolition. <br /> <br />Disconnection of Utilities <br /> <br />The contractor shall be responsible for the disconnection of all utilities. The sanitary sewer shall <br />be capped at the property line with a permanent watertight cap or plug. The water services shall <br />be disconnected at the curb stop. For further specifications on utility disconnections, please call <br />612-717-4050. Water and sewer disconnects must be done by a licensed contractor. Permits <br />will be required for disconnects. Disconnection of all utility services to the site shall be the first <br />order of work. Demolition may not commence until the utility services have been disconnected <br />and a demolition permit has been obtained. <br /> <br />Indemnity and Insurance <br /> <br />The demolition and clearing of the site shall be carried out in such a manner to preclude damage <br />to adjacent properties. The contractor must agree to hold harmless the City of Mounds View, the <br />Mounds View Economic Development Authority, and their respective employees from any <br />claims for damages resulting from any work performed under this contract. <br /> <br />The contractor shall carry not less than $500,000 in public liability insurance and not less than <br />$100,000 in property damage insurance naming the City as an additional insured and agrees to <br />assume all liabilities for any act or deed of said contractor or sub-contractor and shall save the <br />City harmless against any and all suits, actions or claims brought on through any work under the <br />performance of this contract. A certificate of insurance shall be provided at the time of license <br />approval and permit application. <br /> <br />The contractor shall erect and maintain all necessary barricades and warning to adequately <br />safeguard the public and workers. <br /> <br />Notice to City <br /> <br />The contractor shall notify Jim Ericson, Planner, City of Mounds View, two days prior to the <br />commencement of demolition work. <br /> <br />Removal of Materials from Site <br /> <br />All salvage materials and/or debris remaining in the building and on the grounds of the site shall <br />be the property of the contractor and removed from the premises and shall not be stored, burned <br />or buried on the site. The contractor shall completely remove all building materials and debris <br />from the site including all concrete sidewalks, footings, floors on grade and below grade, <br />foundations, curbing, driveway and parking lot areas, unless otherwise specified. No burial of <br />materials will be permitted on the site. <br /> <br />Inspections and Final Grade/Filling <br />
