<br />RESOLUTION NO. 6415 <br /> <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br /> <br />APPROVING A CONTRACT FOR DECISION SERVICES, LTD. TO CONDUCT <br />A COMMUNITY SURVEY <br /> <br /> <br /> WHEREAS, the City Council has determined that the community survey is <br />needed to address specific community issues; and <br /> <br /> WHEREAS, Decision Resources, Ltd., has submitted a proposal to <br />conduct a survey at a cost of $10,200.00 for a 50 question survey; and <br /> <br /> WHEREAS, the expenses related to the survey will be budgeted from the <br />Special Projects Fund; AND <br /> <br /> WHEREAS, the contract will not be signed unless the suggested revisions <br />are made per the City Attorney’s advice. <br /> <br /> NOW, THEREFORE, BE IT RESOLVED, that the City Council approve <br />the agreement with Decision Services, Ltd., to conduct a Community Survey <br />pending revisions made by the City Attorney. <br /> <br /> Adopted this 13th day of December, 2004. <br /> <br /> <br /> <br /> ________________________________ <br /> Jerry Linke, Mayor <br />ATTEST: <br /> <br /> ________________________________ <br /> Kurt Ulrich, City Administrator <br /> <br />(seal) <br /> <br />
