53 <br /> <br />Section 4.03 Bone Marrow/Organ Donation Leave <br /> <br />Employees who work an average of (20) or more hours per week may take up to forty <br />(40) paid hours to undergo a medical procedure to donate bone marrow or an organ. The <br />40 hours is over and above the amount of accrued time the employee has earned. <br />Verification by a physician of the purpose and length of leave is required. If there is a <br />medical determination that the employee does not qualify as a donor, the paid leave taken <br />before the medical determination is not forfeited. Vacation, sick leave and holiday time <br />will accrue during the leave. <br /> <br />Section 4.04 Job Related Injury or Illness <br /> <br />All employees are required to report any job-related illnesses or injuries to their supervisor <br />immediately (no matter how minor). If a supervisor is not available and the nature of injury <br />or illness requires immediate treatment, the employee is to go to the nearest available <br />medical facility for treatment and, as soon as possible, notify his/her supervisor of the <br />action taken. In the case of a serious emergency, 911 should be called. <br /> <br />If the injury is not of an emergency nature, but requires medical attention, the employee <br />will report it to the supervisor and make arrangements for a medical appointment. <br /> <br />Workers’ compensation benefits and procedures to return to work will be applied <br />according to applicable state and federal laws. <br /> <br />Section 4.05 Emergency Leave <br /> <br />In cases of serious emergency, including death or serious illness in the employee’s family, <br />or other emergency of a serious nature, a regular employee’s Department Head may <br />authorize use of sick leave benefits for such absences up to a maximum of three days for <br />each emergency. The number of emergency leaves per employee shall be determined <br />on a case-by-case basis by the City Administrator. For purposes of this Emergency Leave <br />Policy, the term “family” is defined in the Funeral Leave policy below. <br /> <br />Section 4.06 Funeral Leave <br /> <br />A maximum of up to three (3) days off with sick pay will be allowed following the death of <br />a family member, defined as a spouse, domestic partner, son, daughter, father, mother, <br />brother, sister, grandmother, grandfather, father-in-law, mother-in-law, sister-in-law, <br />brother-in-law, daughter-in-law or grandchild. Regular part-time employees will be <br />granted three (3) days (pro-rated) funeral leave, as requested. Additional days may be <br />approved by the Department Head, taken as needed and charged to accrued sick leave <br />and/or vacation. If sick time and/or vacation time is not available, the additional days may <br />be taken as time off without pay. The number of additional days taken off with or without <br />pay will be determined on a case-by-case basis by the City Administrator.
