CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br />1 Carnival License to O'Neil United Shows <br />Mounds View Location Mounds View Square <br />This agreement made and entered into on _ <br />by and between the City of Mounds View and the carnival license <br />applicant, - ----- <br />The license applicant shall hereby be respon:;ible for the <br />following: <br />1. Provide a site plan with an approved location. <br />2. Provide proof of insurance that meets Mounds View <br />Code provisions 116.04. <br />3. Payment of all required electrical permit fees. <br />4. Call Mounds View Inspections, 784-3055, and Fire <br />Department, 786-443G, for inspection of tnachi.ncry, <br />rides and location forty-eight (48) hours prior to <br />operation startup. Approval by the 1luilding <br />Inspector and Fire Dcpar.t:mont musk hi' 1-`( ivcd prior <br />to startup. <br />5. Hours of operation shall be limited from 9:00 A.M. <br />to 10:00 p.m. on Mondays through 'Vhursdays, 9:00 a.m. <br />to Midnight on Fridays and Saturdays, and 12operation: <br />to Midnight on Sundays. Dates of: permitted <br />May 10, 1983 through May 15, 1983__ _____• <br />6. Applicant shall provide other applicable licenses, <br />i.e., food, drink, etc. <br />7.
