39.07 ' <br />dental death to any one person, and subject to the same limit for <br />each person, in an amount not less than $p09;099-98 $500,000.00 on <br />account of any one accident, and property insurance in the amount <br />of not less than $50,000.00. <br />39.08 Duration of License. Licenses shall be issued on an <br />annual basis, with the license year running from July 1 through the <br />following June 30th. <br />39.09 Application for Permits. Application for permits shall <br />be made upon forms provided by the Clerk -Administrator and shall <br />include the following information: <br />(1) The name, address and telephone number of the applicant. <br />(2) The location of the building, structure or lot to which, or <br />upon which, the sign is to be placed. <br />(3) The position of the sign or other advertising structure in <br />relation to nearby buildings, structurc5and streets or highways if <br />necessary. <br />(4) Two blueprints or ink drawings of the plans and specifications <br />and method of construction and attachment to the building or ground. <br />(5) A copy of the stress sheets and calculations may be required <br />by the inspector, on larger signs, showing that the structure is ' <br />designed for dead load and wind pressure in any direction in the <br />amount required by this part and any other applicable law. <br />(6) The name of the person, firm, corporation, or ther organiza- <br />tion erecting the structure, if not the applicant; or the name of <br />the person, firm, corporation or other organization on whose property <br />the structure is to be located, if not the applicant. <br />(7) The written consent of the owner of the property, if not the <br />applicant. <br />(8) A statement as to whether the sign will be illuminated or not. <br />(9) A statement as to whether any electric lights on the sign <br />will be "moving" or blinking. <br />(10) A statement as to whether the sign will be single -faced, <br />double-faced or multi -faced. <br />(11) A statement as to whether or not the copy on the sign will be <br />changeable. <br />(12) A statement as to whether or not the copy on the sign will <br />relate only to the business or activity conducted on the premises on <br />which the sign is to be located. <br />39.10 Permit Fees.
