2KC <br />P R O P O S A L <br />TO: The City of Mounds View <br />C/O Solid Wate Coordinator <br />City of Mounds View <br />2401 Highway 10 <br />Mounds View, Minnesota 55112 <br />In accordance with the advertisement of the City of Mounds <br />View inviting proposals for recycling services herein named, <br />and in conformity with the plans, specifications, and bid <br />pertaining thereto, on file in the office of the Solid Waste <br />Coordinator. <br />(I) (We) hereby certify that (1) (We) (am) (are) the only <br />person(s) interested in this proposal as principal(s); that <br />this proposal is made and submitted without fraud or <br />collusion with any other person, firm or corporation <br />whatsoever; that an examination has heen made of the site of <br />the work and the contract form, together with the plan, <br />specificaions and bid pertaining thereto for the services <br />requested. <br />(I) (We) propose to furnish all necessary machinery, <br />equipment, tools, labor, and other means of collection, in <br />the manner at the time prescribed, all in accordance with <br />the terms of the contract and the plan, specifications, and <br />bid forming a part thereof. <br />(I) (We) further propose to do all extra work which may be <br />required to complete the proposed service, at unit prices or <br />lump sums to be agreed upon in writing prior to starting <br />such work. <br />(I) (We) further propose to execute the form of contract and <br />to fu,nish a contract frond in the amount of the rontract, <br />within ten (10) working days r.f the acceptance of this <br />proposal as secutity for the service aid completion of the <br />work in accordance with the terms of the contract, plans, <br />specifications, and bid. <br />(I) (We) further propose to guarantee all work performed <br />under this contract to be done in accordance with the plans, <br />specifications, and bid in a good and workmanlike manner; <br />and to complete any collection which may have been missed <br />due to defective equipment or oversight. <br />(I) (We) further propose to begin work on or before the <br />first day of June, 198F according to the specifiations and <br />to prosecute said work so as to complete the same as per the <br />contract, plans, specifications and bid. <br />WITNESS <br />By <br />CONTRACTOR <br />J <br />E <br />WITNESS NAME (Tit e) <br />
