Appointment of project director <br />2. Appointment of project staff <br />3. Need determination application Process <br />4. Licensure <br />5. County/provider contract <br />C. <br />A description of how current regional center staff will he used in the <br />delivery of services. This description should include the following: <br />1. Job descriptions of project director and project staff. <br />2. Resumes/vita of the project director and project staff, <br />3. salary data and FPE (full-time equivalency) of staff. <br />D. <br />A statement of measureable project goals and objectives to be accoan- <br />plished including an evaluation format to measure program effective- <br />ness. Program effectiveness for the individual served will be shown by <br />the following: <br />1. Evidence of personal growth and development of individuals <br />receiving pilot project services (e.g., weight, height, verbal <br />ability, self•heip skills, etc.). <br />2. Reduction of psychotropic medications, reduction of seizures, <br />reduction of crisis intervention and evaluation of inappropriate <br />behaviors. <br />3. Evidence of increased family contact. <br />4. Evidence of increased community involvement/participation by the <br />Individual. <br />5. Examples of individual success stories which will be documented <br />(e.g., videotaped). <br />Program effectiveness of the service delivery system will be shown by <br />the following: <br />1. Number of regional center employees involved in the development <br />and delivery of pilot project services. <br />2. Evidence of support of the following: parent, employee unions, <br />advocacy groups, community leaders, etc. <br />3. Evidence that community service building has occurred. <br />E. <br />A description of the project's Internal and external monitoring <br />systems. Indicate how the project will address such issues as quality <br />assurance and licensing. <br />
