-5- <br />0. There will be a plan to assure that services are meeting individual <br />needs and demonstrating, program effectiveness. It is the responsi- <br />bility of the county of financial responsibility to monitor services <br />provided to the individual to ensure that services are provided in <br />accordance with the person's individual service plan as described In <br />Minnesota Rules, Parts 9525.0015 to 9525.0145 (Emergencyj. <br />P. Projects will participate in a standardized evaluation process. <br />0. All services will follow existing licensing processes and standards. <br />R. Projects will be funded to allow for an average per diem expenditure <br />over the project perind of no more than $100 per day total cost per <br />individual. Project funding shall pay for "home and community -based <br />services"* as described in Minnesota Rules, parts 9525.1800 to <br />9525.1930. Board and lodging costs will be paid for through SSI/MSA <br />funding. <br />S. A plan to disseminate project development and service delivery infor- <br />mation to other regional centers and counties, parent organizations, the <br />legislature, advocacy groups, other providers, and state agencies. <br />Information shared would include positive and negative experiences and <br />recommeadatl.ons. <br />Requests for any variance to the proposal criteria listed above must be <br />accompanied by a ratiogale for the variance. Variances must be approved <br />c; the assistant commissioners prior to March 1, 1986, to be eligible <br />for project funding. <br />' *"Uume and community -based services" means the following services which <br />are provided to persons with mental retardation, if the services are <br />authorized under United States Code, title 42, section 1396 et. seq., <br />and authorized under the waiver granted by the United States Department <br />of Health and Human Services: case management, respite care, home- <br />maker, in -home family support services, supported living arrangements <br />for rhildren, supported living arangements for adults, day habilita- <br />tion, and minor physical adaptations to the home, as defined in part <br />9525.1860; and other home and community-hased services authorized under <br />United States Code, title 42, section 1396 et. seq., if approved for <br />Minnesota by the United States Department of Health and Human Services. <br />VT. FORMAT <br />Reptonnl center project proposals for the delivery of community integrated <br />services for regional center residents include: <br />A. A description of the proposed community integrated services project <br />which 1s based nn the proposal criteria listed under AIV in this re- <br />quest for proposal. The proposal must show how these criteria are or <br />will he met in service delivery. This description should include plans <br />to decertify lvds to accomplish legislative mandates and, if necessary, <br />meet requirements of the HCISch vs. Levine Consent Decree. <br />
