Phase II Goal Setting / Team Building <br />The general results of the goal setting are anonymously shared to form a ^} <br />common base of information and understanding. The specific agenda is I <br />tailored to especially meet the identified needs and time frames specified, <br />but the general framework follows this format: <br />s Share and clarify data from interviews <br />• Identify critical issues and concerns <br />• List and clarify goals <br />• Prioritize goals <br />• Develop Action/Implementation Plan <br />r Evaluation and Follow-up Strategies <br />Phase III Follow-uo and Evaluation <br />Following the workshop, the client writes a written report of the workshop. <br />It is recommended to hold another session within 3-6 months of the event, <br />where progress on the goals is recognized and a reassessment can be made. <br />It is important to emphasize that planning is an on -going process. Although <br />this program should result in significant progress, it is just the beginning <br />of continuous planning process. <br />
