Phase lI Goal Setting / Team Building <br />The general results of the goal setting are anonymously shared to form a <br />common base of information and understanding. The specific agenda is <br />tailored to especially meet the identified needs and time frames specified, <br />but the general framework follows this format: <br />• Share and clarify data from interviews <br />a' Identify critical issues and concerns <br />0 List and clarify goals <br />• Prioritize goals ' <br />• Develop Action/Implementation Plan <br />s Evaluation and Follow-up Strategies <br />Phase III Follow-up and Evaluation <br />Following the workshop, the client writes a written report of the workshop, <br />It is recommended to hold another sess;on within 3-E months of the event, <br />where progress on the goals is recognized and a reassessment can be made. <br />It is important to emphasize that planning is an on -going process. Although <br />this program should result in significant progress, it is just the beginning <br />of continuous planning process. <br />
