-2- <br />04 10. initiate and control all supplies and equipment purchases to replenish existing <br />inventories and provide additional items necessary for effective program <br />implementation and maximum safety for all participating players. <br />11. Establish necessary policies and procedures required to insure all required <br />paperwork is maintained for the proper and efficient running of the athletic <br />program including preparation of well documented reports that clearly identify <br />existing program activity and extent of participation. The reports should <br />include supporting information identifying accountability for administration of <br />each program and related employee lists, budget allocadons, etc. (Year end <br />report, budget report, quarterly public class evaluations.) <br />In return, for the aforementioned services from the City of <br />Mounds View hereby agrees to the following: <br />1. Provide reimbursement for the proposed activities at an annual rate of $10,000 <br />per year to be paid in twenty-six equal increments. <br />2. The work schedule will include four hours per day of office time, 5 days/week. <br />3. The City will provide office space at the City Hall offices. <br />4. The status of this position, according to the personnel code, is regular part-time <br />incurring PERA, workers comp and social security deductions.
