-2- <br /> ID <br /> PRODUCT LIABILITY INSURANCE <br /> The successful bidder shall provide the City of Mounds View, at or before the time of <br /> delivery of the product, with a certificate of product liability insurance with liability limits of <br /> not less than one million dollars ($1,000,000) naming the City of Mounds View as an <br /> additional insured and loss payee with respect to the entire unit bid including, but not <br /> limited to, all parts and attachments thereto (whether the said parts and attachments are <br /> supplied directly by the successful bidders or by subcontractors to the successful bidders). <br /> PROPOSALS <br /> The bidder shall submit his proposal on forms as furnished in these specifications. Only <br /> proposals which are made out on the original form attached hereto will be considered, and <br /> no bid will be accepted without completion of the attached questionnaire. <br /> INTENT <br /> It is the intent of the City not to be restrictive to any one manufacturer. However, the unit <br /> desired must meet the enclosed standards, and any unit bid which does not comply with <br /> the aforementioned specifications should be noted. All items appearing in the bidder's <br /> regular specifications, which are in addition to the standards are assumed to be included <br /> in the bidder's proposal. <br /> MANUFACTURER'S SPECIFICATION <br /> A complete set of a manufacturer's specifications and illustrated descriptions shall be <br /> furnished with the bid. <br /> DELIVERY <br /> The unit shall be delivered to the City of Mounds View Public Works Garage, 2466 <br /> Bronson Drive, Mounds View, Minnesota, 55112, at no additional compensation. <br />
