ANTHONY, MINNESOTA <br /> 1982 INSURANCE BID SPECIFICATIONS <br /> GENERAL CONDITIONS <br /> (A) Explanation to Bidders <br /> Any request for explanation regarding the meaning or interpretation of these <br /> specifications may be directed to David M. Childs , City Manager, 3301 Silver <br /> Lake Road, St. Anthony, Minnesota 55418, phone 612-789-8881 , and if time <br /> allows, should be set forth - in writing. Each bidder should submit any <br /> explanations concerning their bid proposals or interpretations in the form <br /> of an addendum attached to the bid proposal documents furnished herein. <br /> Oral explanations and interpretations made prior to the bid opening shall <br /> not be binding. <br /> (B) Bidders Understanding <br /> It is expected that bidders shall inform themselves as necessary and will <br /> make themselves familiar with City properties to ascertain by their inspec- <br /> tion any pertinent local conditions. <br /> (C) Bid Requirements <br /> • A bid deposit in the amount of five percent (5i) of the total bid price <br /> must accompany each bid proposal . The bidder,- at their option , shall <br /> furnish either a cash deposit , certified check or _bond as security in the <br /> amount required. All checks must be made payable to the City of St. Anthony. <br /> All securities will be retained until either a bid has been awarded or 30 <br /> days time has elapsed from the date of the receipt of bids. Bids shall be <br /> submitted on the forms provided by the City and erasures or alterations <br /> therein may void bid proposal . <br /> Bidders must execute and submit with their bid the Affidavit of Insurance <br /> Agent,. the Affidavit of Non-Collusion, and evidence of Insurance Agents/ <br /> Brokers Errors & Omissions Insurance with a minimum limit of $250,000. <br /> Bids will not be considered without the accompanying deposit and documents. <br /> The bid deposit is subject to forfeiture in the event the successful <br /> company or companies does not perform on its contract. <br /> (D) Rating <br /> The. insuring company or companies must be a Minnesota admitted or approved . <br /> surplus lines company having a general policy-holders rating of "A" or <br /> better and a financial rating "AAAA" or better in the 1981 edition of <br /> "Best' s Insurance Guide". Mutual companies must be non-assessable; or, if <br /> assessable, a policy from an approved insurer which insures the full amount <br /> of contingent accessability of the City will be considered. <br /> • <br />
