PAUL, MINNESOTA 5511.7 <br /> DEF!'NITIONS OF PROPOSED SKRVlCO3 <br /> The North Suburban Developmental Achievement Center, Inc. intends to provide <br /> a training program for mentally retarded persons l8 months to 4 years of <br /> age and for those persons 2J years and older. These persons may also have <br /> cerebral palsy or another developmental disability. Some of the children <br /> may be considered developmentally delayed . <br /> It is also North Suburban's intent to provide a quality training, program <br /> reared to the individual needs of each person enrolled. Likewise, we would <br /> like to be able to serve all the persons in our service area without a <br /> waiting list . <br /> The goal of preschool services is to prccyide qual icy developmentr:al training <br /> to each child according; to their .ind.iv:idu:i.l needs. fj4 <br /> The objectives of preschool are: <br /> 1 . Children are referred from the Community. We will provide evaluation <br /> or refer them to an outside agency in order to determine if our <br /> services are appropriate for the child ; <br /> 2 . Develop programming opportunities in areas of communication skills , <br /> fine and gross motor skills , self-help skills , social-emotional <br /> skills and cognitive skills; _ <br /> 3. Continual assessment and monitoring of progress by utilizing the <br /> Hawaii , Gesel le, REEL, IEDP, and 7.immt rraan scales: <br /> 4. Follow-along for children who gradune before the age of 4 years; and <br /> 5. Refer children to a community pro; rnm whon rl iVihle or when another <br /> program may he more appropriaLu. <br /> The methods by which the objectives of preschool are carried out are by: <br /> 1 . Utilizing certified/qualf ied staff and appropriate staff/participant <br /> ratios; <br /> 2. Classroom training, on a one-to-one basis or in small groups ; <br /> 3. Therapists also use a one-to-one approach or sm0 1. groups ; <br /> 4. Annual assessments with quarterly gnal.s written into the individual <br /> program plan; <br /> 5. Parental involvement in the development of the I . P.P. , homevisits, <br /> classroom observation, occasional doctor Qv: luntion) visits and <br /> a close working relationship with the local school. districts. <br /> • <br />
