• THE MINNESOTA COUNTIES INSURANCE TRUST <br /> DRUG AND ALCOHOL TESTING PROGRAM <br /> PARTICIPATION AGREEMENT <br /> THIS PARTICIPATION AGREEMENT provides for the delivery of drug and <br /> alcohol testing services by the Minnesota Counties Insurance Trust (MCIT) for the <br /> City of St. Anthony, " Member.' <br /> 1. SERVICES TO BE PROVIDED <br /> MCIT agrees to provide services to the Member in accordance with the drug and alcohol <br /> testing service provider contract or successor contract, the current contract is attached <br /> • hereto and incorporated herein as Exhibit 1. MCIT is not obligated to provide such <br /> services should the drug and alcohol testing service provider contract be terminated or <br /> cancelled. <br /> 2. SERVICE FEES <br /> The Member will compensate MCIT in accordance with the fee schedule in ,ARTICLE IV <br /> of Exhibit 1. MCIT will submit invoices to the Member on a monthly basis for services <br /> performed under this Agreement.
