AGREEMENT BETWEEN THE CITIES OF BLOOMINGTON <br /> AND ST. ANTHONY FOR PROVISION OF HEPATITIS B VACCINE <br /> This Agreement made this day of , 1991, by <br /> and between the City of Bloomington ('Bloomington") and the City of St. Anthony ("St. <br /> Anthony"); <br /> WHEREAS, Hepatitis B is an infection of the liver caused by the hepatitis B virus; <br /> and <br /> WHEREAS, each year approximately 300,000 persons in the United States are <br /> afflicted with hepatitis B, more than 10,000 persons require hospitalization and an <br /> estimated 250 people die from the disease; and <br /> WHEREAS, persons who may be at risk of acquiring hepatitis B due to their job <br /> responsibilities include such governmental employees as nurses, paramedics, law <br /> enforcement personnel, and firefighters; and <br /> WHEREAS, a key strategy for preventing the spread of hepatitis B is to vaccinate <br /> individuals with a high risk of infection; and <br /> • WHEREAS, Bloomington, through its Division of Public Health, has agreed to <br /> vaccinate employees of approximately 24 municipalities in order to minimize infection with <br /> hepatitis B; <br /> NOW, THEREFORE, the parties hereto, in consideration of the covenants <br /> thereinafter set forth, agree as follows: <br /> 1. Bloomington, through its Division of Public Health, will administer the <br /> Hepatitis B vaccine to any employee of St. Anthony who consents to be <br /> vaccinated. Each employee requesting immunization shall be required to <br /> sign a Consent Form, which is attached hereto and marked Exhibit A, prior <br /> to being vaccinated. <br /> 2. Bloomington shall administer three (3). separate vaccines for each St. <br /> Anthony employee. <br /> 3. St. Anthony shall pay Bloomington the sum of One Hundred and Forty <br /> Dollars ($140.00) for each employee consenting to be vaccinated. On or <br /> before a date to be determined by Bloomington, St. Anthony shall remit to <br /> • Bloomington the total amount of compensation from all employees of St. <br /> Anthony who have consented to be vaccinated.
