The Police Department shall provide the Safety Coordinator with <br /> appropriate information, (copies of relevant forms, etc.), on all <br /> accidents reported. <br /> 2. Vehicle accidents which need not be reported to the State shall be <br /> referred to the employee's supervisor and the Safety Coordinator. <br /> MOTOR VEHICLE <br /> ACCIDENT REPORTING PROCEDURE <br /> A. All vehicle accidents invoMng on-duty City employees who are operating, assisting <br /> or responsible for a motor vehicle must be reported to the employees supervisor <br /> and local police agency. Each accident shall be reported on a Police Traffic <br /> Accident Report. If the local police agency procedure prohibits the reporting of <br /> such accident on the Police Traffic Accident Report, contact the St. Anthony Police <br /> so that such report can be completed. <br /> B. If the accident occurs in the City of St. Anthony, a Police Officer shall be contacted <br /> to complete the necessary Police Traffic Accident Report. <br /> C. In the event of a motor vehicle accident the employee shall contact his supervisor <br /> as soon as practical. The supervisor shall, if practical, respond to the location of <br /> the accident and aid in obtaining the preliminary report. <br /> D. All department supervisors shall forward a copy of all accident reports to the <br /> Safety Coordinator as soon as possible. <br /> A GUIDE TO DETERMINING PREVENTABILITY OF ACCIDENTS <br /> NON-PREVENTABLE ACCIDENTS <br /> A. Struck in Rear by other Vehicle <br /> Non-preventable if: <br /> 1. Driver's vehicle was legally and properly parked. <br /> 2. Driver was proceeding in his own lane of traffic at a safe and lawful speed. <br /> 3. Driver was stopped in traffic due to existing conditions or was stopped in <br /> compliance with traffic sign or signal or the directions of a police officer or <br /> other person legitimately controlling traffic. <br /> 4.
