Occupational Accident: shall be regarded by the City, as any unintended <br /> or unexpected work related occurrence resulting in: <br /> 1. personal injury <br /> 2. and/or damage to property, equipment or vehicles, excluding <br /> normal wear and breakage inherent in proper operation of <br /> such equipment or vehicles. <br /> B. Occupational Injury: shall be regarded as any injury such as a cut, fracture, <br /> sprain, amputation, etc., which results from a work related accident or from <br /> exposure involving a single accident in the work environment... (includes <br /> conditions resulting from animal bites, or from one time exposure to <br /> chemicals, etc.). <br /> RESPONSIBILITY: <br /> A. Employee: <br /> 1. The employee shall report all accidents and illnesses, (including <br /> vehicular accidents), to the supervisor as soon as practicable. The <br /> employee must fill out the necessary report(s) to be turned into the <br /> supervisor. <br /> 2. The employee shall report all work related vehicular accidents to the <br /> St. Anthony Police Department as soon as practicable. <br /> 3. In addition, the employee shall report to his/her supervisor potential <br /> safety problems or current violations. <br /> B. Supervisor: <br /> 1. Upon learning of an occupational injury or illness, the supervisor <br /> shall ensure that a "First Report of Injury" form is completed within <br /> one working day. The supervisor must sign this form. <br /> 2. Upon learning of a work related non-injury accident involving <br /> property, equipment or vehicles, the supervisor shall ensure that a <br /> "Preliminary Accident Report" is completed. <br /> 3. Extremely minor injuries, i.e. very small cuts, scratches, etc, should <br /> be noted by the supervisor.
