25 <br />Anthony and Roseville neighborhoods tend to be much older, with fewer children at home, much <br />more likely to be in households headed by a woman or in a one-person household. <br />For the proposed project, Access Solutions will partner with Greater Metropolitan Housing <br />Corporation (GMHC) to identify individuals targeted for services. GMHC is active in both of the <br />targeted cities, managing construction projects and a range of home improvement programs. <br />Through its Housing Resource Center program, GMHC administers the MIIFA Fix up Fund loan <br />program and the MHFA Deferred Rehab loan program for very low-income households. In <br />addition, both communities offer city -funded low-interest home improvement programs managed <br />by GMHC, and in St. Anthony Village there is also a rebate program that returns a portion of the <br />cost of improvements to the homeowner. <br />Through preliminary conversations GMHC and Access Solutions staff, city officials in St. <br />Anthony Village and Roseville have expressed interest and willingness to participate in the <br />proposed home modification project. Both cities value the presence of elders in their <br />communities and are committed to promoting aging in place. <br />A portion of grant funds received will be used to pay for minor modifications (up to $500) for <br />participants. For larger modifications, individuals will receive assistance in accessing additional <br />funds and project management services through the Housing Resource Center. <br />4. Project Goals, Outcomes, Activities (Implementation Plan): <br />The goal of this project is to facilitate and promote aging in place by providing outreach, <br />education and home modification services to older adults in the suburban St. Anthony Village <br />and Roseville neighborhoods of the Twin Cities. <br />Project objectives include the following: <br />1. To provide a home safety assessment, modification recommendations and <br />implementation of minor modifications (up to $500) for 25 households in each of the two <br />cities each year. <br />2. To train up to five city staff at St. Anthony Village and Roseville, and five staff of <br />Housing Resource Centers on universal design, aging in place and working with older <br />adults. <br />Access Solutions staff will conduct a thorough home access assessment for each client to <br />determine how they function in their home. This assessment will focus on the safety and <br />accessibility of both the interior and exterior of the home. We will also determine if there are <br />health issues, diagnoses or long-term limitations for the older adult that need to be considered in <br />the scope of the project. Staff will then recommend modifications to be implemented to address <br />short and long term functional needs. This process will include a summary report of <br />recommendations, options to consider and the benefits of making each modification. In addition, <br />we will address the needs of caregivers, when appropriate, to facilitate safe care giving. <br />For each of the 50 households receiving assessment services, the project will provide up to $500 <br />towards minor home modifications (equipment and labor).
