26 <br />expensive modifications will receive assistance in applying for funding assistance through the <br />loan programs administered by GMHC, or if ineligible, assistance in identifying and accessing <br />other resources. Access Solutions' standard fee for a home modification assessment is $250. For <br />this project, funds from the state will make it possible to provide the assessments free of charge. <br />Access Solutions staff will provide full project management services to ensure the project's <br />success. Project management services include but are not limited to education/prevention, <br />modification/assessment report, modification recommendations and design solutions, product <br />specification, bid prep/gathering, installation/construction coordination and funding source <br />assistance. Follow-up consultations will ensure that all modifications and equipment are being <br />used correctly, safely and meet the needs of the client. <br />To ensure sustainability after the project concludes, we will conduct comprehensive training <br />sessions for city staff (city planners, inspectors, senior center staff, etc.) and for GMHC staff <br />(construction managers) on universal design, aging in place and working with older adults. This <br />will provide sustainable resources for them to assist residents in the future. During the first year <br />of the project Access Solutions staff will provide all assessments. The second year will involve <br />Housing Resource Center staff conducting a portion of the assessments as a part of their training. <br />We will also provide at least two educational sessions for consumers about universal design and <br />aging in place in each of the two cities. <br />3. Evaluation Plan: <br />For each participant in this project, we will conduct survey after their home improvements have <br />been completed. The survey will contain questions pertaining to the project itself and how the <br />process went. We will also ask questions pertaining to improvements in the quality of their lives <br />resulting from the project and if it truly allowed them to age in place. Follow-up calls will be <br />made 60 days after completion of modifications and six months to capture additional <br />information. For each person served in this area in any capacity, we will track the age and <br />income level of individuals served, number and type of modifications completed, and referrals to <br />grant programs or other funding sources for assistance with larger modification needs. <br />Evaluation will also include surveys of staff at both cities and GMHC regarding changes in their <br />level of awareness and knowledge base to better assist residents. GMCFI will assist in tracking <br />the accessing of loan programs as a result of the project. <br />
