RESOLUTION 2014-13 <br />APPROVING A SITE PLAN FOR NATIONAL SENIOR HOUSING SERVICES, <br />LLC, FOR A SENIOR HOUSING FACILITY ON THE PROPERTY LOCATED <br />NORTH OF 145TH STREET AND WEST OF FINALE AVENUE NORTH <br />WHEREAS, National Senior Housing Services, LLC, requested approval of a site plan <br />for a senior housing facility on the property legally described as; <br />Lot 1, Block 1, Lavalle Fields <br />WHEREAS, the Planning Commission has reviewed said site plan on April 24, 2014, and <br />recommends approval subject to the conditions listed in this resolution. <br />NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF <br />THE CITY OF HUGO, MINNESOTA, that it should and hereby does approve the site <br />plan for National Senior Housing Services, LLC, for a senior housing facility with the <br />following findings: <br />1. Approval is contingent upon City Council approval of the Final Plat for Lavalle <br />Fields. <br />2. The applicant shall meet all requirements in Resolution 2013- 3 and 2013-5 for <br />Lavalle Fields Preliminary Plat and Planned Unit Development (PUD). <br />3. All items on the Engineer's memo and "red -line" detailed engineering comments that <br />have been sent directly to the applicant's engineer dated April 18, 2014, shall be met. <br />4. The development shall be consistent with the plans submitted on April 11, 2014 and <br />April 14, 2014. <br />5. The applicant will be responsible for maintaining stormwater facilities and vegetation <br />proposed as a part of the project. A maintenance agreement between the City and the <br />applicant shall be reviewed and approved by the City then recorded against the <br />property prior to the start of grading activities on the site. <br />6. The irrigation system shall be designed and installed for it to be connected to the <br />large stormwater pond planned north of the property as part of phase 2 of the Lavalle <br />Fields PUD. At the time the stormwater pond is constructed, the system shall be <br />converted to irrigate the property with stormwater. <br />7. The applicant shall be required to apply for formal site plan approval for all future <br />phases to be considered by the Planning Commission and City Council. <br />8. The monument sign location shall be removed from the public right of way. The <br />monument sign shall be designed using materials consistent with the building <br />materials. <br />
