-3- <br />3.0 QUALIFICATIONS <br />3.1 For those jobs so designated by the City Council, an annual physical <br />examination demonstrating fitness for the job may be required by the <br />City at City expense. Failure to satisfactorily pass such physical <br />examination may be grounds for termination. <br />3.2 Any offer of employment on a permanent full-time basis may be con- <br />tingent upon satisfactory completion of a pre-employment physical <br />examination to be performed at City expense. <br />4.0 RULES AND REGULATIONS <br />4.1 The rules and regulations of the <br />of employees and shall be used ' <br />of any employee or termination o <br />f <br />5.0 PERFORMANCE APPRAISAL <br />department shall govern the conduct <br />�n determining just cause and suspension <br />employment. <br />5.1 The job performance of each permanent employee shall be appraised <br />each year by theenployee's immediate supervisor. This appraisal, <br />which shall be carried out in a manner designated by the City Council, <br />shall become a part of the personnel record of the employer for a <br />period of five (5) years. The appraisal shall be reviewed with the <br />employee, and any comments made by the employee, shall become a part <br />of the record. Appraisals shall be developed, reviewed, and filed <br />in December of each year. In the case of department heads or other <br />employees reporting directly to the City Council, the Mayor shall <br />generate the performance appraisal. <br />6.0 TERMINATION <br />6.1 Vacation that is earned, but not taken and accrued, will not be paid <br />for, unless the resigning employee has given two (2) weeks notice. <br />7.0 INSURANCE <br />7.1 The City will pay the difference between the workman's compensation <br />hourly benefits and the average of the employee's salary for the <br />last six (6) months for a period of 180 days. <br />8.0 DISABILITY <br />8.1 In the event of an employee's disability, certified by a licensed <br />medical doctor, and after 14 continuous days of absence, the City <br />will compute the average gross salary for the prior 180 working <br />day period, and will pay, to the employee, 2/3's of the average <br />for a period not to exceed six (6) months or $3500.00 maximum. <br />ADOPTED .AND PASSED THE 7th DAY OF November , 1983. <br />C <br />.
