Johnson, Director <br />Washington County Department of Public Health and Environment <br />RE: Community Partner Project Funding <br />Washington County Department of Public Health and Environment is pleased to announce an <br />opportunity for cities, townships, and communities to receive Community Partner Project <br />funding as part of the Statewide Health Improvement Program (SHIP) grant. SHIP is an integral <br />part of Minnesota's nation -leading 2008 health reform law, strives to help Minnesotans lead <br />longer, healthier lives by preventing the chronic disease risk factors of tobacco use and exposure, <br />poor nutrition and physical inactivity. Many Americans live in places where getting the <br />recommended amount of physical activity is difficult; in most communities, even the most trivial <br />errand requires use of a vehicle. <br />Funds will be awarded to aid in increasing opportunities for non -motorized transportation and/or <br />increasing access to community recreation facilities. Funding is competitive and eight <br />applications are expected to be awarded. <br />Funding requirements: <br />• One application per city or township <br />• Applicants may request up to $4,000 <br />• Proposal must be submitted by a City or Township located in Washington County <br />• Actions must be geared toward sustainable changes in policy, systems, and/or <br />environments <br />• Actions must address physical activity (an obesity prevention strategy) <br />• The pre -application form is due October 1, 2010 <br />• SHIP funds must be spent by June 30, 2011 <br />• Applications must be received by October 29, 2010 at 4:30 p.m. <br />Consideration will be given to applications with: <br />• Changes available to the maximum number of community members, diverse or <br />low-income populations; or <br />• Collaboration between cities, townships, and community organizations, i.e. <br />schools, parks etc. <br />A detailed list of fundable projects is included in the pre -application form. <br />
