Johnson, Director Washington County Department of Public Health and Environment RE: Community Partner Project Funding Washington County Department of Public Health and Environment is pleased to announce an opportunity for cities, townships, and communities to receive Community Partner Project funding as part of the Statewide Health Improvement Program (SHIP) grant. SHIP is an integral part of Minnesota's nation -leading 2008 health reform law, strives to help Minnesotans lead longer, healthier lives by preventing the chronic disease risk factors of tobacco use and exposure, poor nutrition and physical inactivity. Many Americans live in places where getting the recommended amount of physical activity is difficult; in most communities, even the most trivial errand requires use of a vehicle. Funds will be awarded to aid in increasing opportunities for non -motorized transportation and/or increasing access to community recreation facilities. Funding is competitive and eight applications are expected to be awarded. Funding requirements: • One application per city or township • Applicants may request up to $4,000 • Proposal must be submitted by a City or Township located in Washington County • Actions must be geared toward sustainable changes in policy, systems, and/or environments • Actions must address physical activity (an obesity prevention strategy) • The pre -application form is due October 1, 2010 • SHIP funds must be spent by June 30, 2011 • Applications must be received by October 29, 2010 at 4:30 p.m. Consideration will be given to applications with: • Changes available to the maximum number of community members, diverse or low-income populations; or • Collaboration between cities, townships, and community organizations, i.e. schools, parks etc. A detailed list of fundable projects is included in the pre -application form.
