GENERAL PROVISIONS (Continued) <br />Insurance: MN Services has in force the following policies: <br />Public Liability, Property Damage, Workers Compensation and <br />Blanket Fidelity Bond. Certificates will be mailed to you upon <br />request directly from our insurance provider. <br />Adjustments: The price set forth in this proposal is based on <br />the necessary labor, supplies, equipment and supervision <br />necessary to maintain your facility properly as set forth by the <br />areas serviced and the frequency of the work. In the event of <br />any major changes in such conditions, the contract may be <br />either increased or decreased accordingly with thirty (30) days <br />written notification. These adjustments shall be by mutual <br />agreement. <br />Terms: Should this proposal be accepted, it will form the basis <br />of the agreement and will continue in effect from year to year, <br />except that it may be modified from time to time as set forth <br />above. The contract may be terminated in its entirety by either <br />party at any time by giving notice in writing of their desire to <br />do so at least ninety (90) days prior to the selected date of <br />termination. A longer period may be effected should you <br />require this. <br />
