Stakeholder Involvement Gather input and build local support for the project with neighborhood residents May 2019 <br />and businesses. This will include one-on-one meetings with the surrounding <br />property owners to review and discuss the potential trail improvements and <br />options. Surrounding residential neighborhoods will be notified. <br />Feasibility Design <br />Memorandum of Understanding: <br />A feasibility study for the trails will be prepared based on the outcome of preferred <br />trail routes and types to include a 30% design document package illustrating trail <br />layout and partial grading plan with an estimate of probable costs. An <br />accompanying report/memorandum will be prepared. The preferred trail layout <br />plan will be brought to a public Parks Commission meeting for review and <br />approval. The preferred trail layout plan will be brought to a City Council meeting <br />for review and approval. <br />In -kind <br />June 2019 1 $9,500 <br />• By completing this action plan and signing the County service contract we agree that any equipment purchased with Statewide Health Improvement Program <br />(SHIP) funding will be used for the intended purpose of this action plan. <br />• Partners are expected to spend at least 50% of contracted funds by June 30, 2019 to allow for implementation and evaluation of action plan items as is <br />required by the grant. <br />• Project lead will complete invoices and reimbursement forms while ensuring action plan activities are being completed. <br />• Project lead will participate in informal monthly check -ins, one site visit and complete the Final Evaluation Report by October, 2019. <br />*Refer to the guidelines for list of allowable expenses. <br />If you need assistance due to disability or language barrier please call 651-430-6001 (TTY 651-430-6246) <br />AN EQUAL EMPLOYMENT OPPORTUNITY / AFFIRMATIVE ACTION EMPLOYEE 12/18 <br />$10,000 <br />
